病因分析需更精准：癫痫灶定位困难是重要原因，约 40% 患者的癫痫灶位于大脑深部（如丘脑、海马）或存在多灶性发作（如双侧大脑半球均有癫痫灶），常规脑电图难以捕捉；病因未去除包括脑部弥漫性病变（如脑白质营养不良）、遗传代谢性疾病（如苯丙酮尿症），这类患者即使药物治疗，也难以控制发作；药物耐药则与患者基因相关，如 ABCB1 基因表达升高，会导致药物在脑组织中的浓度降低，影响疗效。此外，治疗不规范（如药物剂量不足、频繁更换药物）也可能导致 “假性难治性癫痫”，约占难治性癫痫的 15%，这类患者通过规范治疗可恢复疗效。
